Addressing an Oral-Antral Communication
Following extraction of Maxillary Molars, there is a risk of developing an oral-antral communication depending on the proximity of the Schneiderian membrane to the roots. Most of the time, a small perforation can be repaired with a collagen plug or matrix. Once in a while, healing is compromised and a large communication develops (1,2). It is important to address this as soon as possible by sealing off the communication from the oral cavity in order to avoid developing a sinusitis. Periosteal release of the buccal flap for passive flap advancement is one of the techniques to use as a solution (3,4).
