A fit and healthy woman in mid forties presented with chronic sinus from upper right central incisor. The central and lateral had been damged in accident as child and had undergone root filling, post crown and apicetomies with retrograde root fillings (amalgam) x2 each. Last apicectomies approx ten years ago. The central had obviously failed but lateral was asymptomatic and there was no buccal plate tenderness. She has no medical problems, good oral hygiene, high smile line and high expectations. My plan would be to remove central, allow to heal and 6 weeks later place implant +/- bone augmentation. But I am worried lateral is unstable and could compromise implant but if I take central and lateral out and resore with single implant in central position am I oevrtreating. Second question is regarding these chronic sinus cases does anyone have experience of socket preservation when socket is infected, if it can work what is success rate and what are the tricks, I am thinking maybe pre-treat with systemic antibiotics for one week then remove tooth, irrigate and pack socket with Bio-oss and collagen plug on top??
