exposed threads

Hi group. Looking for some suggestions here. Implant placed as immediate into lateral incisor position about 8 months ago. Patient is a smoker and presently feels some tenderness to palpation of the gingiva on the buccal cervical area of the implant. Implant is 15 mm long and is well integrated. No exudate is present. There is no discomfort when the implant itself is touched. As you can see there is bone loss around the top 3 threads. My thoughts are that the facial threads of the implant are exposed causing the tenderness. I have heard different opinions includiing: 1) do nothing 2) flap and remove granulation tissue, clean implant, place bone and membrane and close. 3) flap and polish exposed facial threads. I an leaning towards 2 above based on our discussion here. If I do this, how long should I wait until restoring the tooth? Does anyone have a preferred bone grafting material for this? Has anyone used DynaBlast here? Thanks in advance for your help Craig
